Home Support



At Dementia Matters, we offer an all-inclusive Home Support service providing anything from a 1 hour call up to bespoke 24 hour packages of care designed around you in your home.

With 25 years’ experience in Home Support and access to over 30 qualified and experienced Home Support Staff, 2 expert coordinators and a Registered Manager who also holds 25 years’ experience in the care sector we are sure we can meet your care requirements.

Using the latest technology in care planning and coordination, our assessments are completed with compassion and dignity, our care planners help gain a detailed and in depth knowledge for the kind of care you would like to receive and how you would like it delivered.

Forward care planning helps us to build a picture of how you wish for your care to develop during your journey, keeping your wishes at the forefront of our service. It’s important the care you receive is the care you want, care that you trust.

We aim to provide dedicated teams of either three or four care staff, to ensure that we can provide continuity of care and cover for sickness and holidays. Your care and the service you receive will then be monitored at regular intervals’.

 

Tasks we can do:

  • Support with personal care e.g having a bath, getting dressed etc
  • Medication prompts and administrating if necessary
  • Attending Hospital or GP appointments
  • Support to do weekly shopping
  • Support to go out and meet friends or visit hairdressers or just someone to chat too
  • Activities within the home this could include reading local newspaper, doing the gardening, doing the housework or laundry
  • We can also do end of life care looking after people in their own homes until the end of life with support from District Nurses etc
